When checking in at the front desk, we will require that you present your insurance card with each visit. If you are unable to provide proper insurance information you will need to pay all charges in full on the day services are provided. If your insurance requires a co-pay it will be collected at the time of service.

If you are unable to keep your appointment, please give us a 24 hour cancellation notice to avoid a $50.00 no show fee.
