| Lakewood Family Medicine is very excited to announce that it has been accepted to participate in a 3 year Pilot Program to become certified as a Patient-Centered Medical Home. This program, one of the first of its kind in the country, redirects the focus of health care delivery back onto the patient. Studies have shown that this type of care not only improves outcomes but saves money too. Our focus is to increase the patient satisfaction!
